Continuity mistake: When Klaw is boarding the helicopter, the door is open, and the interior is colored brown. After Klaw boards, the door closes, and then instantly turns the same brown color as the interior, making it look like the door disappears into thin air as soon as it closes. (00:14:54)

Revealing mistake: When Tony Stark is approaching his father Walter in the hospital bed, what looks to be a hair on the animation cel appears to the left (viewer's right) of Walter's head, beginning on the pillow and extending off the bed, and then disappears after a few frames, just before Tony steps into the shot. Plot hole: After Psycho-Man, disguised as Reed, lures Sue away from the outdoor tables of the cafe where she was dining with Alicia, Johnny and the real Reed walk up to Alicia a mere three seconds later and ask where Sue is. Reed and Johnny should have easily seen Psycho-Man impersonating Reed and walking away with Sue, because they approached Alicia from the street, it's not like they turned the corner out of some alley. Continuity mistake: When Reed is showing everyone the new element, he is wearing a white lab coat. When Sue tells him to take a night off to go to the movies, he is suddenly wearing a light-blue suit jacket, and Sue holds up a white lab coat in front of him like the one he was just wearing. Continuity mistake: When Walter Stark is first shown in the hospital bed, his pillow is brown, and looks to be made from a hard material, judging from the light reflection coming off it. When the shot changes, the pillow is beige and looks soft like a pillow should. (00:07:19)

Continuity mistake: When Tony is asking his father if he remembers anything about Nick Fury, he leans up against a control panel, there is a large monitor behind him, and he is standing with his left profile to his father. When the shot changes, it is now a bare wall behind Tony, and he is directly facing his father. A couple of shots later, Tony is suddenly standing several feet away from his father, nowhere near the control panel. (00:10:41)

Stupidity: At the beginning of the episode, Pyro is among the Genoshan prisoners, and he is wearing his flamethrower device. While his inhibitor collar will suppress his mutant power of pyrokinesis, he can still use the flamethrower itself as a weapon. This would be akin to a prison allowing an inmate to walk around with a loaded gun. (00:02:12)

Continuity mistake: When Dum Dum Dugan is holding up the documents for Iron Man to see, his right hand is grasping them by the top-center. When the shot changes to the documents reflected in Iron Man's helmet, Dugan's hand is missing from the reflection. The words on the document are also gibberish. (00:03:12)
