Factual error: During the initial car chase scene, the speedometer of the stolen car is shown a couple of times. You can see that the speedometer is in kilometers per hour, not miles per hour. This is because most of the movie was actually filmed at a studio in Rome, Italy, so the cars they used were built with European specs.

Continuity: When the black criminal dies from an injury caused by the huge explosion at the mid river passage, Kit is leaning against the front bumper of a burnt out car, but a few shots later when the camera cuts to a wider angle, the car has vanished and all we see is the old man, Kit and the criminal on the floor with no car, but in the following shot the car reappears again. This has nothing to do with camera angles.
Continuity: When the tunnel caves in as a result of the explosion, the cars about to enter it come to a stop in almost a straight line. When Stallone gets out of his limo, the cars are now scattered everywhere. Then, when he goes back later to the collapsed entrance, the cars are not only in different positions, but some have changed in make and model as well.
