Continuity: EP "Goodbye to All That": On the firing range when John Connor fixes the stoppage on the rifle, his shooting glasses are on his head, when he hands the rifle back, the glasses are now over his eyes, and in the next shot they are on his head again. Not enough time has occurred between shots for this to happen.

Continuity: Ep. "Samson and Delilah" - All shots of the Jeep Cherokee prior to it crashing (and during the process of flipping over) show that it wears factory-optional silver steel wheels of a 'star' style. But as the wreck settles upside-down, it's wearing plain steel wheels (like those found under full wheel covers or on a spare tire).
Continuity: Ep. "Samson and Delilah" - When the Jeep Cherokee flips, it lands on the driver-side/rear corner of its roof, completely collapsing that corner of the roof. The camera angle changes and instantly the Jeep is landing on both of its driver-side tires, roof intact, and rolls onto its left side. The angle changes again, and the collapsed roof is back as the Jeep slides a bit more before the shot changes again, and the undamaged roof is shown as the Jeep rolls onto its roof and comes to a stop. The camera pans around the rear of the wreck, and there is no roof collapse at either rear corner.
Continuity: Ep. "Samson and Delilah" - As Derek sits at the table, Sarah is digging jelly from a jar with a knife, moving it onto toast that sits on a plate on the table. The shot instantly reverses and the jar is on the table and she's spreading the jelly with the knife on the toast she now holds in her hand.
Factual error: Ep. "Samson and Delilah" - When Cameron is disabled by electric shock, Sarah and John race to remove her CPU. As John is doing the work, Sarah is monitoring the passing time because Cameron's system will restart after 120 seconds. But after 19 seconds pass, Sarah says 70 seconds are left. 7 more seconds pass and she states 55 seconds remain. After a total of 34 seconds have passed, Sarah states there are 35 seconds left. 2 seconds later, 30 seconds are left. In all, 50 seconds have passed and Cameron wakes up as John and Sarah run away, having given up, meaning they were counting time properly, yet they obviously weren't.
Pilot (season 1, episode 1)
Continuity: As the FBI agent shows his ID to Dixon, it is in his right hand, the badge end up. He holds a folder in his left hand. The camera cuts away for just a moment, then returns to show the ID in his left hand, badge to the right, and the folder is in his right hand, all without enough time to make the switches.
Continuity: As Cromartie is approaching the Dyson home, he carries an automatic rifle with his right arm. As he makes a turn toward the left, he lowers the rifle down to his side, pointing it at the ground. He hears a car start and an instant camera angle change shows the rifle is still pointed forward and upward, not down, with no time for the change.
Queen's Gambit (season 1, episode 5)
Continuity: As Cameron is questioned about the girl that commited suicide, the interviewer holds a photo of one of the mostly-black posters. When we see his view of the photo, the white letters spell 'DAN', but before and after that shot, when we see his face, we can see through the photo, and it only reads 'A', which is a different poster seen earlier in the same episode.
Character mistake: When Dixon arrives to find Derek lying on the table with a serious gunshot wound to the chest, he immediately begins giving Derek chest compressions - without doing even the most basic assessment of Deker's condition, verifying whether he is breathing or even has a pulse. Dixon is a trained Emergency Medical Technician, not a passing samaritan. His error is against the most basic rules of his profession.
Vick's Chip (1) (season 1, episode 8)
Continuity: As the scene begins, we are looking through Cameron's eyes as she holds up a photo of a night cityscape framed by plants. She lowers the photo to reveal she is standing in the exact spot that the picture was taken. In both the photo and her view, the plants that frame the distant city are very well lit. But the shot changes to show Cameron turning to face camera and we see the plants are now in near-total darkness beyond her.
