ER (1994)

Continuity: As Mark futilely pounds on Jodi O'Brien's chest, Carol can be seen in the background, removing her protective glasses with one hand. However, after the time of death has been called and they're showing the reactions of everyone present in the room (Susan, Carol, Carter, Lydia, Chen), Carol is seen taking the glasses off again, this time with both hands.

Other: Season 12, Episode "Blame it on the Rain": At the beginning of the episode, Abby is almost run over by a man driving an SUV. They exchange words and then Abby throws a supposedly full cup of coffee at him. The cup of coffee bounces off the seat of the man's car and it is obvious that the cup is empty.

Continuity: When Malik finds Carter's family noted amongst the richest in Chicago, he says that Carter's father is named 'Roland'. However, in later episodes, Carter's father's name is given as John Truman Carter, Jr.

Doug Ross frequently hung his head low, appearing ashamed or thoughtful or privately amused, depending on the scene. This wasn't just an element of the character: George Clooney had taken to writing his lines on papers, sheets, and other props.

Every episode has the patient list by the nurse's station. On the patient list, different names are mentioned, like Marguiles or Tierney. They put these names there as a spoof of the show.

The first four episodes of the show all began the same way - with one of the doctors being awakened early in the morning from 'Exam 8' at the end of the hallway, in this order: Greene (Pilot), Lewis (Day One), Benton (Going Home), Carter (Hit and Run). Ross (George Clooney) was the only one of the original five who never slept in Exam 8.
