Continuity: When Dana enters the hall after being attacked by Pin Head, the door opens out into the hall, but it should open into the room, not out into the hallway.

Continuity: Toward the end when Alex is being punched by the "Puppet Master" his mouth is all bloody. Then Megan smashes a vase on The Puppet Master's head, and in the next shot Alex's face is no longer bloody. In the next closeup of him the blood has returned.

Revealing: When Dana is walking down one hall with Leroy and a bottle of champagne, the exit sign is reversed.

Continuity: When Andre Toulon puts Blade into the suitcase, near the start of the film, Blade's hook is on his right hand but it is attached to his left hand in all other shots of him.

Continuity: When Frank and Carissa are having sex and Carissa notices the door is open, her hair moves from hanging over her shoulder to behind her shoulder.

Plot hole: We discover that Teresa the maid has been turned into some kind of living zombie at the end of the movie. She confronts Alex and Megan near the end, and they run away from her and then fight with the Puppet Master. The issue of Teresa is then never addressed in the rest of the film, she just simply disappears from the plot and the scene.

Revealing: When Alex and Megan find the dead bodies in the dining hall, in the closeup shot of Dana, her neck pulsates.

Visible crew/equipment: Toward the end of the movie, when Andre Toulon is trying to escape the elevator through the top, the wires for the Tunneler Puppet are visible in one shot.
