| 1 | The Simpsons | This is a mistake for the introduction for the later episodes. When Homer screams, he turns round. You see this in a wide shot. There are no boxes to the right of the door in the garage. However, two just appear out of thin air when he runs through the garage. |
| 2 | Friends | When Joey is counting the floors of Ross' building (the first time) to find out where Hot Girl lives, he is wearing a black top with a zip. When he rushes over to Ross' building he walks down the hall and knocks on what he believes to be Hot Girl's door. In these shots he is wearing a dark purple button up shirt. As soon as Ross answers the door, Joey is wearing the black top with a zip again. |
| 3 | Sex and the City | At the beginning of every show we see Carrie walking down the street and she gets splashed by the bus. First the bus has people on it, then in the next shot they all disappear. |
| 4 | Buffy The Vampire Slayer | When Giles is giving his speech about the Hellmouth the books Buffy's holding keep changing from facing him to facing Buffy and back again. |
| 5 | 24 | When Nina's just been shot, and makes it to the barn to call CTU (around 7.10am), while she's speaking on the phone there's a cameraman visible on the right hand side (DVD). |

| 6 | Family Guy | Brian's newspaper's name changes from "Daily Informant" to "Daily Tim...". The newspaper also turns upside down between the two shots. |
| 7 | Scrubs | In the apartment there is a cardboard box on a table, with a strip of blue tape on it and a tube of paper sticking out of it. A few shots later another strip of blue tape suddenly appears on the box and the tube of paper has vanished. |
| 8 | Lost | When Hurley is at the new house, after his mom hurts her ankle, he reaches into the car window to get the cell phone. When he begins to dial, the cameraman's arm moving about is reflected in the back chrome of the side view mirror. |
| 9 | Smallville | Clark's hands on the hood of Jeremy's truck shift in different shots from spread wide to in front of him and close together. |
| 10 | Will & Grace | When Jack holds his glass of water, he changes from holding the stem of the glass, to the actual bulbous part of the glass between shots. |
| 11 | The X-Files | The first time Scully walks into Mulder's office, we see his "I want to Believe" poster in full view. Later on in the scene, we see papers stacked in front of it. |
| 12 | Seinfeld | After Jerry, Elaine, George and Kramer walk out of the orange stairwell, there is a shot as they come out the door. A car drives past. The crew is reflected in the window. |
| 13 | Battlestar Galactica (1978) | When the Warriors are playing cards, Starbuck places his cards on the table near the gold coins, saying "A perfect pyramid. Unless there's a better hand, the pot is mine". His right fingers tap on the table and his cigar is in the ashtray. When the shot cuts, his hand is suddenly up by his mouth, holding the cigar, and the cards and coins have vanished. |
| 14 | Mr. Bean | In the later intros (when Mr Bean falls into a spotlight in a town) part of Mr Bean's foot isn't in the spotlight, but it still has a shine. |
| 15 | Charmed | In the scene where Prue comes home and finds Andy inside the manor. There is a shot where if you look behind Andy's shoulder you can see half of the body of the director in the background in the observatory room sitting in a director's chair. |

| 16 | Futurama | When Fry and Bender are in the museum and locked in the "Villains" room, Bender breaks the two girders in the window, and then his arms falls off. As he puts his arms back in, you can see behind him that the girders are back in place in the window. |
| 17 | Angel | At the end of the opening credits, you can see Angel's reflection in the puddle on the floor. Since he is a vampire, this should not happen. |
| 18 | Red Dwarf | Right after the shrinking boxers scene, Kryten says the small boxers are missing from the bunk, but you can see them right in front of him. |
| 19 | Stargate SG-1 | Carter and Cadet Haley enter the Stargate and we see them through the "puddle." When they come out on the other side, their positions are reversed. |
| 20 | Frasier | When Frasier and Niles are eating the steaks at the restaurant that Martin brought them to, in some shots you can see Frasier's baked potato, and in others from the same angle it is not visible. |
| 21 | Stargate: Atlantis | When they first arrive on Teyla's planet, the DHD is close to the gate. When the wraith attack, the DHD is much further from the gate. |
| 22 | King of the Hill | After Hank and Bobby return from La Grunta, Hank parks the car halfway into the garage. Yet when Peggy washes the blood from the deer off the front of the truck, the truck is now on the driveway and not near the garage. |
| 23 | The O.C. | When Ryan is lighting Marissa's cigarette with his, at the start of the shot it's full, then the shot goes to Ryan and it's still full. But then the shot goes to Marissa and if you look at Ryan's cigarette you can see it is almost all gone. Then the shot goes to Ryan and the cigarette is full again. |
| 24 | Star Wars: Clone Wars | Season 1 Episode 3: Chapter III. After the ARC Troopers have destroyed the Droid tank, the Captain waves the team on. In that shot, the red coloration of his helmet's chin is missing (it is white instead). |
| 25 | The Big Bang Theory | When Sheldon is telling Leonard that he didn't ask Penny out, there is a yellow food-bag on the counter between them that keeps changing it's position with every shot change. |
| 26 | Blackadder | During the introduction, the snake tries to get away from someone who is supposedly Blackadder. He gets the snake. It's head is above a black tile. There is then a close-up of the snake, Blackadder wags his finger at it. It is now above a white tile. |
| 27 | Gilmore Girls | Luke uses a red screwdriver trying to get something out of the toaster and leaves the screwdriver in the toaster while he goes to get coffee. When it cuts back to Lorelai, the screwdriver is on the counter, beside the toaster. |
| 28 | Heroes | S02E09 - When Claire runs away from Bob after he calls her "Ms. Bennet", you can see Bob's shadow and another person's shadow on the ground. Since no one else was standing next to them, it has to be a crew member, most likely a camera man based on the odd shape of the shadow. |
| 29 | M*A*S*H | When Radar and Burns leave the tent for morning parade, Radar's clipboard acquires a pencil. |
| 30 | Xena: Warrior Princess | As the people of Troy are about to be attacked by the warriors in the horse, the camera pans up to the side of the wall where Deiphobus is talking with another soldier. You can see a crew member crouching down behind them wearing a white and grey shirt. |
